An innovative skin cancer treatment, integrating immunotherapy with mRNA vaccine technology, has demonstrated promising outcomes in an advanced trial, according to pharmaceutical giants Merck and Moderna. The combined therapy, named intismeran, outperformed immunotherapy alone in decreasing melanoma recurrence and preventing cancer spread post-surgery, the companies reported. The treatment’s interim analysis displayed statistically significant and clinically relevant improvements.
Medical professionals cautiously welcomed the news, emphasizing that the Phase 3 trial, a precursor to regulatory approval, remains ongoing. Dr. Marcus Butler, a melanoma specialist at Toronto’s Princess Margaret Cancer Centre, expressed optimism, citing positive results from an earlier Phase 2 study. He emphasized the need for more substantial benefits, although it is still early in the process.
Upon completion, if the trial data validates the findings, the treatment’s availability in Canada could be more than eight months away, noted Dr. Ramy Saleh from McGill University. Scaling up a novel treatment globally presents logistical challenges that require collaboration among governments, healthcare entities, and the industry, Saleh added.
The treatment pairs Merck’s pembrolizumab immunotherapy drug with a personalized mRNA vaccine from Moderna, targeting specific mutations in patients’ tumors. Pembrolizumab activates the immune system by removing inhibitory checkpoints, while mRNA vaccines instruct cells to combat pathogens, including cancer cells.
The trial involved 1,137 high-risk melanoma patients, with Canadian participants receiving either Keytruda plus the individualized vaccine or Keytruda alone. Notably, no new safety concerns arose during the trial. However, concerns exist regarding the potential immune stimulation from mRNA vaccines and the study’s limited focus on early-stage melanoma patients.
Despite the positive interim results, Butler cautioned that the treatment’s efficacy in later-stage melanoma cases remains uncertain, as past trials have yielded mixed outcomes. Further research is necessary to determine its effectiveness across different melanoma stages.
